Blank Collection Worksheet

The Blank Collection Worksheet prints an empty field-sampling form, for when no order precedes

the work and no phone is available. It is filled in by hand and entered afterwards.

Row countHow many empty sample rows the sheet will carry.
Collector namePrinted in the header, so the paper knows who is holding it.
DateThe sampling date.

Connects to

  • Samples — where the rows end up once entered.
  • Field collection — the digital road to the same work. The sheet is the fallback when a phone

is not an option, not the norm.

  • Sampling Plans — the sheet does not replace the plan; §7.3 asks how the sample was chosen,

regardless of whether it was written on paper.

If this is missing

  • 🔴 Paper is not a system record. Until the rows are entered, those samples do not exist: no

code, no worklist, invisible to everyone.

  • The gap between collecting and entering is the risk. Anything entered later than it happened

may need a late-entry reason (ALCOA+ contemporaneous). Enter the same day wherever you can.

  • With no collector name on the sheet, attributability is lost the moment somebody else types

the rows in.
